Preservation Rhinoplasty with Dr. Aaron Kosins

Preservation rhinoplasty is not just a technique I perform—it is a philosophy that I have helped develop, refine, and teach across the United States and internationally. I have dedicated my career to advancing rhinoplasty, and preservation rhinoplasty represents one of the most important evolutions in modern nasal surgery.

The Three Core Components of Preservation RhinoplastyI truly live and breathe rhinoplasty. It is my passion, my focus, and the area of plastic surgery where I have invested the majority of my academic, clinical, and research efforts.

In 2013, I helped advance rhinoplasty through the adoption and teaching of ultrasonic (piezoelectric) instrumentation, which transformed how surgeons approach nasal bone surgery. Shortly after, I was at the forefront of introducing and teaching preservation rhinoplasty in the United States, helping to shift the paradigm from traditional reductive techniques to a more refined, structural, and preservation-based approach.  I wrote the first article on Preservation Rhinoplasty in the world, and published several chapters in all three editions of the Preservation Rhinoplasty book.  Recently, our book Structure Preservation Rhinoplasty was published and I was one of the lead co-editors.

Today, preservation rhinoplasty is widely recognized and taught globally, and I continue to contribute through research, publications, and educational meetings held here in Southern California.

Elevating Aesthetic Surgery

What Is Preservation Rhinoplasty?

Preservation rhinoplasty is a modern approach to rhinoplasty in Newport Beach, CA that focuses on preserving the natural structures of the nose while reshaping them, rather than removing and reconstructing them.

Traditional rhinoplasty has typically followed a “reduce and rebuild” philosophy. In contrast, preservation rhinoplasty follows a “preserve and reshape” approach.

This technique is built on three key principles:

  • Preservation of the nasal bridge (dorsum)
  • Maintenance of the tip cartilages with minimal excision
  • Elevation of the soft tissue envelope in a specialized surgical plane

By maintaining the natural anatomy of the nose, this approach can produce results that appear more natural, function effectively, and age more predictably over time.

The Three Core Components of Preservation Rhinoplasty

1. The Skin Sleeve (Soft Tissue Envelope)

One of the most important advancements in preservation rhinoplasty is the elevation of the skin and soft tissue over the nose.

Traditional techniques often use a sub-SMAS plane, which may lead to:

  • May involve increased swelling
  • Numbness
  • Prolonged healing
  • Soft tissue thinning over time

Preservation rhinoplasty utilizes a subperichondrial-subperiosteal plane, which allows for:

  • May help reduce postoperative swelling
  • May support faster recovery of sensation
  • Minimal scar remodeling
  • Improved long-term soft tissue integrity

This method has been supported by imaging and clinical research and contributes to smoother recovery outcomes.

2. Dorsal Preservation (The Bridge of the Nose)

In traditional rhinoplasty, removing the dorsal hump creates an “open roof,” requiring reconstruction and repositioning of nasal bones.

With dorsal preservation rhinoplasty, the natural structures of the nasal bridge are maintained, and the dorsum is reshaped by modifying the underlying septum.

This allows surgeons to:

  • Preserve natural dorsal aesthetic lines
  • Minimize disruption to nasal anatomy
  • Create smoother, more natural contours
  • Reduce the need for aggressive reconstruction

3. Tip Preservation (Alar Cartilages)

The nasal tip is one of the most complex aspects of rhinoplasty.

Traditional techniques often involve cutting and removing cartilage, which may lead to:

  • Loss of structural support
  • May help reduce the risk of tip deformities
  • Unnatural long-term appearance

Preservation rhinoplasty focuses on:

  • Maintaining the integrity of the alar cartilages
  • Using advanced suturing techniques
  • Supporting structure with minimal excision

This results in a more stable, natural-looking nasal tip with fewer long-term complications.

Benefits of Preservation Rhinoplasty

Patients choosing preservation rhinoplasty in Newport Beach, CA often experience:

  • More natural-looking results
  • May help reduce swelling and bruising
  • May support a faster recovery compared to traditional techniques
  • May support preservation of nasal function
  • Long-lasting structural integrity
  • Lower risk of long-term deformities

Because the technique works with the patient’s natural anatomy rather than removing it, outcomes are often more predictable and stable over time.

Who Is a Candidate for Preservation Rhinoplasty?

Preservation rhinoplasty may be ideal for patients who:

  • Have a dorsal hump they wish to reduce
  • Prefer a natural, non-operated appearance
  • Are seeking primary rhinoplasty
  • Have favorable nasal structure and anatomy

Not all patients are candidates. During consultation, nasal anatomy and aesthetic goals are evaluated to determine whether preservation rhinoplasty or a structural approach is more appropriate.

Real Patient Transformations

Preservation Rhinoplasty vs. Traditional Rhinoplasty

Traditional Rhinoplasty:

  • Removes dorsal hump
  • Requires reconstruction
  • May be associated with increased swelling and tissue disruption
  • Necessary for certain complex cases

Preservation Rhinoplasty:

  • Maintains natural structures
  • Reshapes rather than removes
  • Often results in smoother healing
  • Produces more natural-looking outcomes

A customized approach is always used to align surgical technique with the patient’s anatomy and goals.

Recovery After Preservation Rhinoplasty

Recovery after preservation rhinoplasty is typically easier and faster than traditional rhinoplasty.

Most patients can expect:

  • A nasal cast for five to seven days
  • May help minimize bruising and swelling
  • Return to work within seven to ten days

Although early recovery is quicker, final refinement continues over several months, with results improving for up to one year.

Most patients return to normal activities within 7–10 days, with continued refinement over several months.

Preserving natural structures often leads to more stable, long-lasting outcomes.

In some cases, yes. More complex asymmetries may require structural techniques.

It is generally less disruptive to nasal anatomy, leading to reduced swelling and faster healing.

Costs vary depending on anatomy and surgical complexity. A personalized quote is provided during consultation.
